2022 Women's PGA Cup Volunteer Registration October 26-29

• Please note we are accepting only female volunteers for this all-women event
• If you have date or time-of-day limitations or health issues which limit your duties, you may write in any special instructions in the "additional info" field below and the co-chairwomen will work with you when scheduling, closer to the event
• Committee assignments cannot be guaranteed, and volunteers may be switched if necessary
• Please read this entire section below prior to payment
  
  By paying the volunteer registration fee you hereby agree to the Waiver/Participation statement and acknowledge you will be available for the one-day training on Wednesday, October 26 at 2:30 PM
  
  I wish to participate as a volunteer at the 2022 Women's PGA Cup. I understand I am not entitled to the volunteer opportunity and that I will abide by all the rules and regulations adopted and published by the PGA relating to volunteers and the operation and conduct of the 2022 Women's PGA Cup and use of the facilities provided by the 2022 Women's PGA Cup. I understand that my failure to observe these rules may result in my being excluded from volunteering at the 2022 Women's PGA Cup.  I represent that I am physically able to participate as a volunteer at the 2022 Women's PGA Cup. I fully understand and agree that my participation at the 2022 Women's PGA Cup may entail the risk of physical injury. I agree to assume such risk and to waive any claim of any kind whatsoever, whether resulting from an injury or otherwise rising from my participation at the 2022 Women's PGA Cup.
  
  I further agree to release, indemnify, and hold harmless the 2022 Women's PGA Cup, Twin Warriors Golf Club and their respective directors, officers, employees, agents, an/or representatives from any and all liability occurring as a result of my participation the 2022 Women's PGA Cup. As a volunteer, I will be personally responsible for any financial cost incurred as a result of my participation at the 2022 Women's PGA Cup, including, without limitation, transportation, or medical expenses incurred as a result of any injury. I understand that the 2022 Women's PGA Cup and Twin Warriors Golf Club assume no liability of lost, stolen, and or damaged personal property and I hereby agree to release the 2022 Women's PGA Cup and Twin Warriors Golf Club from any such liability. 
  
  I, the purchaser, agree that I am at least 18 years of age and have read and voluntarily agreed with this waiver/participation form.
  
  All volunteers who have paid their fee will receive a PGA logo shirt and cap, access to the grounds all week, breakfast and lunch on your shift days and an invitation to attend the Opening Ceremony. Volunteers who work a minimum of two shifts on two separate days will receive one (1) round of golf including cart, at Twin Warriors.
